This form may be required by the state when you apply for your Learner’s Permit and License. You will need fill in the Date of Instruction for every hour completed, but note that per state regulations, you may NOT exceed 2 hours of “Classroom Instruction” per day.

The basic in-car observation and behind-the-wheel session log to be completed by the instructor.
The 30 hours of behind-the-wheel practice must be completed in the presence of an adult who meets the requirements of Section 521.222 (d)(2), Transportation Code before the young driver is eligible for a provisional license.

Per the state, any applicant, ages 16 & 17, applying for a Texas Driver License must show the certificate of completion for the ITTD Program. Note that the certificate MUST be dated within 90 days of their Drive test.

Other Forms

This form is required by DPS as part of the permitting and licensing process. If you are enrolled in a Public or Private school, you may request a VOE from your school’s administration office. If you are homeschooled, you may download this form and have your designated school teacher fill it out.
If you choose to transfer your completed Classroom hours to a commercial school, you’ll need to have your “Parent/Instructor” check the box on the top portion of the DE964 Certificate that reads “Transfer (See Transfer Form for Details).” Note that per the State of Texas, you may only transfer 6 or 32 hours of Classroom Instruction.
The Graduated Driver License (GDL) program was implemented January 1, 2002 to allow new drivers the opportunity to improve their driving skills over time and in lower-risk environments.
The Parent Taught Driver Education – Designation Form is required by the Texas Department of Public Safety (DPS) if anyone other than the student’s parent or legal guardian has been “designated” to teach this course. The Designated instructor” must be noted on the Parent Taught Driver Education (PTDE) Program receipt that must be purchased prior to beginning our course.
